Design Services on a Sliding Scale?

My wife is a therapist and it is highly encouraged in her profession to offer such services on a sliding scale, meaning that depending on a clients income, the rate at which services are offered is adjusted to more closely match what they can afford.

This is encourages those that may not have the means to seek such services to do so. I'd like to explore the possibility of offering design and drafting services on a sliding scale as well. This seems like a natural thing to considering many of us have heard or even said ourselves, "everyone deserves good design". But in fact, most design professionals offer such services at inflated rates, and I believe a good percentage of people might never consider hiring a designer, architect, interior designer or landscape architect because of this fact.

So what do you think, can this concept work and still turn a profit?
